Changing your contact details

To update details like your name, address, phone numbers or email address:

Patients are responsible for ensuring that their contact details are accurate and kept up to date. Maintaining current information helps us communicate important updates about your healthcare, including appointments, results, and correspondence. If any of your details change, please notify us promptly by either submitting a change of contact details form here, submitting an administrative request through our online Triage system or by visiting the surgery to complete an amendment form at reception.
 

Change of address

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

Changing your phone number or email address

You will need to show us identification before we can change your phone number or email address.

Changing your date of birth or name

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
