Charges for non-NHS services

We are an NHS practice and provide all NHS services to our registered patients for free.  We are able to provide a small range of non-NHS services as well but for these a small charge will apply.  These charges are as follows:

Certificates and forms

Private Sick Note  £15.00

Driving Licence  £15.00

Fitness to Travel £20.00

Occupational Health Forms  £20.00

Holiday cancellation  £35.00

Private Health Forms  £35.00

OFSTED/Police/Fire £35.00

Certificate of incapacity £20.00

Insurance claim form  £35.00

Seat belt exemption certificate  £20.00

Vaccination certificate  £10.00

Medical Examinations and Reports

HGV/taxi/diving/adoption medical £100.00

Report on proforma without exam  £40.00

Report detailed opinion without exam  £75.00

Other non-NHS work will be charged at a rate appropriate to the amount of time the GP has to spend on it.
